You agree that Your information given to Us is a true and accurate record. You agree to be responsible for all the payment of fees and understand that payment is due at the time of service. Any expenses, costs of disbursements incurred by JT Dental shall be paid by the responsible party listed in the details You have given. You further acknowledge that failure to attend any appointment without notice may result in a fee and a deposit requirement for any future appointments (see below for details). You have read and agree with the privacy statement below. Medical History Forms must be filled out every 12 months as per the Australian Dental Association guidelines. These will be scanned and saved to your patent file digitally. You agree to this process.

Changes to your appointment

While we endeavour to provide patients with a courtesy reminder for their appointments, it is ultimately the patient’s responsibility to remember the appointment time. Our practice requires 48 hours notice for changes for all appointments. Failure to give adequate notice may result in a rebooking fee.

Storage and security of your information

We receive and store information you enter on our website or give us in any other way from time to time. This will only apply if you have registered or subscribed to such publications by registering your details with us. Live Chat

Our Live Chat module is provided by Facebook, a chat platform that connects users with the support team for JT Dental.

We are collecting names and telephone numbers only with the consent of the users, in order to start the chat and follow up on the enquiry. JT Dental is not making use of these messages or data other than to follow up on users’ registered issues or inquiries. Your personal data will be processed and transmitted in accordance with the General Data Protection Regulation (GDPR).
