Any expenses, costs of disbursements incurred by JT Dental shall be paid by the responsible party listed in the details You have given. You further acknowledge that failure to attend any appointment without notice may result in a fee and a deposit requirement for any future appointments (see below for details). You have read and agree with the privacy statement below. Medical History Forms must be filled out every 12 months as per the Australian Dental Association guidelines. These will be scanned and saved to your patent file digitally. You agree to this process.

Changes to your appointment

While we endeavour to provide patients with a courtesy reminder for their appointments, it is ultimately the patient’s responsibility to remember the appointment time. Our practice requires 48 hours notice for changes for all appointments. Failure to give adequate notice may result in a rebooking fee.

We receive and store information you enter on our website or give us in any other way from time to time. Individual profile and JT Dental details are not used for any other purpose. Details are only supplied to a third party supplier when it is required by law, for goods or services which you have purchased or to protect our copyright, trademarks and other legal rights.
